To request a change for your outlet address , please complete the form below.
Once received, one of our team members will get in touch with you to follow up on the request.
Please have the following documents prepared for verification:
1. A copy of your ACRA Business Profile (dated within the last 6 months)
2. Photo of Store
- If store is open, provide photos of the outlet showing the shop's name, unit number, and the display of goods/equipment
- If store is not open, copy of renovation permit and ensure that it reflects the new address
